Care Coordination

Summit Medical Consultants works with the hospital admitting you or your loved one to the Skilled Nursing Facility or Long-Term Care Facility to ensure seamless medical care. We are in contact with the hospitalist through a check-out process and our providers review your case thoroughly upon admission. A provider from Summit Medical Consultants will see you within 72 hours of admission and regularly as needed throughout the time you are admitted to the facility to ensure your rehabilitation is progressing, and your acute and chronic conditions are appropriately managed. We work with social work, case management, physical, occupational, and speech therapies to develop a plan of care for you.

 

Upon discharge from the facility, we will send clinical documentation to your Primary Care Physician and are available as a resource to them if they have questions about your inpatient stay. We will set up your initial Home Health orders, if applicable, so that you can continue therapies without interruption and transition back to your PCP.
